For patients with severe obesity, the Modified Duodenal Switch, also called Laparoscopic Single Anastomosis Duodenal-Ileal bypass with Sleeve or SADI-S, is the most effective surgery for achieving significant, long-term weight loss.
This bariatric surgery may be performed as the primary surgery or as a sleeve gastrectomy revision when the gastric sleeve results are insufficient.
Although duodenal switch surgery has been for more than fifty years, it was not until the refinement of the SADI-S during the past few years that the procedure received endorsements from the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) and the American Society for Metabolic and Bariatric Surgery (ASMBS).
With the surgical assistance of the DaVinci robot system, Dr. Nicole Basa and Dr. Alan Abando offer modified duodenal switch surgery (SADI-S) to patients who desire significant weight loss and resolution of obesity-related conditions such as type 2 diabetes, obstructive sleep apnea, and imbalanced blood fats.
Laparoscopic Single Anastomosis Duodenal-Ileal bypass with Sleeve is a two-part surgery most easily explained as a combination of gastric bypass surgery and sleeve gastrectomy surgery.
In the first part, a large portion of the stomach is removed to create a much smaller and narrower stomach than the original (sleeve gastrectomy). The reduction of stomach size restricts the quantity of food that can be eaten before feelings of fullness are experienced.
The second part of the procedure consists of bypassing a section of the small bowel, thereby reducing the amount of surface available for absorbing nutrients and calories.
In combination, the new anatomy restricts the amount eaten and reduces the absorption of calories from the small amount of food consumed.
Although the SADI-S surgery for weight loss simplifies the original duodenal switch surgery, it is still a complex surgery with more opportunity for complications than other bariatric surgeries. In addition, there are also the consequences of long-term inadequate vitamin absorption and other protein and fat malabsorption issues. Therefore, you should discuss the mandatory lifestyle changes after this bariatric surgery with your weight loss surgeon.
Decreased appetite: Reducing the stomach size restricts how much food can be consumed comfortably in a single sitting.
Rapid weight loss: Restricting stomach size and altering absorption results in faster weight loss than other surgical procedures.
More significant long-term weight loss: The SADI-S surgery allows for almost 100% excess body weight loss after two years.
Reduced obesity-related health problems: SADI-S is excellent in reducing the problems of obesity’s co-morbid conditions**.

Duodenal switch surgery is a 2-part procedure. The first part begins with a gastric sleeve to make the stomach smaller (this helps patients eat less and feel more full). The second part involves bypassing part of the small intestine to reduce the caloric absorption of food.
